پيشگفتار

در طول سال‌های اخیر شاهد افزایش هشدار دهنده‌ای در میزان گزارشات خطاهای دارویی و آسیب‌های ناشی از آن در بیماران بوده‌ایم که این افزایش مبین به خطر افتادن امنیت بیماران می‌باشد. خطاهاي دارويي ممکن است در يکي از پنج مرحله تجویز دارو توسط پزشک (Prescribing)، انتقال دستور پزشک از پرونده به کاردکس دارویی و درخواست دارو توسط منشی یا پرستار به داروخانه از طریق سیستم
HIS (Hospital Information System)
بیمارستان (transcribing) و یا ثبت نسخه الکترونیک توسط پزشک، آماده کردن دارو توسط تکنسین داروخانه و چک توسط داروساز (dispensing)، مصرف دارو برای بیمار توسط پرستار/ کمک بهیار (administering) و یا خود بیمار و مانیتورینگ دارو (monitoring) رخ دهد.
اگر چه تنها بخشی از خطاهای دارویی تبدیل به گزارش می‌شوند ولی همین تعداد نیز باعث آگاهی بیشتر اعضای تیم سلامت شده تا ضمن شناخته شدن این خطاها از بروز مجدد آنها در شرایط مشابه جلوگیری نمایند. وجود فرایندهای پیچیده در نسخه پیچی و توزیع دارو ضرورت بررسی دقیق این فرایندها را دو چندان نموده است. بنابراین تلاش برای ترغیب اعضای تیم سلامت در گزارش این حوادث از جمله اهداف هر بخش مراقبت‌های دارویی باید باشد.
به دليل تنوع زيادي که در روش‌ها و ميزان گزارش خطاهاي دارويي وجود دارد تعيين دقيق ميزان بروز خطاهاي دارويي امري دشوار است. علاوه بر تنوع وسيع در موارد مذکور، عدم انجام يک بررسي سازمان يافته با يک متد طراحي شده مناسب و عدم انجام بررسي‌هاي کافي در زمينه خطاهاي دارويي باعث عدم قابل اطمينان بودن بررسي‌هاي انجام شده در این زمينه مي‌شود. در این کتاب سعی شده با معرفی و ارائه مصادیق خطاها بخصوص در حوزه دارویی و معرفی راه‌کارهای پیشگیری از آنها گامی در راستای افزایش ایمنی بیماران در حوزه درمان برداشته شود.

Preface

In recent years, we have seen an alarming increase in the amount of reports of medication errors and the resulting injuries in patients, which indicates that the safety of patients is at risk. Medication errors may occur in one of the five stages of prescribing medication by the doctor (prescribing), transferring the doctor’s order from the file to the drug cardex, and requesting medication by the secretary or nurse to the pharmacy through the system.
HIS (Hospital Information System)
The hospital (transcribing) or recording the electronic prescription by the doctor, preparing the drug by the pharmacy technician and checking by the pharmacist (dispensing), taking the drug for the patient by the nurse/paramedic (administering) or the patient himself and monitoring the drug (monitoring) occur. .
Although only a part of medication errors are reported, but this number has also made the health team members more aware so that these errors can be recognized and prevented from occurring again in similar circumstances. Existence of complex processes in screw prescription and drug distribution has doubled the necessity of careful investigation of these processes. Therefore, trying to encourage health team members to report these incidents should be one of the goals of every pharmaceutical care department.
It is difficult to accurately determine the incidence of medication errors due to the great diversity in the methods and amount of medication error reporting. In addition to the wide variety in the mentioned cases, not conducting an organized investigation with a properly designed method and not conducting sufficient investigations in the field of medication errors causes the unreliability of the investigations conducted in this field. In this book, by introducing and presenting examples of errors, especially in the pharmaceutical field, and introducing solutions to prevent them, a step has been taken in the direction of increasing the safety of patients in the field of treatment.
